OCCUPATIONAL HEALTH SERVICES PROGRAM

SCOPE OF SERVICE:

Health Index, Inc. offers a vast array of services that are available to area business and industry. Our Occupational Health Services Program is customized to suit the individual needs of the specific company. Health Index welcomes the opportunity to meet with company representatives to conduct a company needs assessment, which will then be utilized in developing an individualized program for your company.

Health Index, Inc. offers the following services:

Pre-Employment and Periodic Physical Examinations - This flexible service will be custom designed based on your company's needs assessment. The level of each exam will take into account the stressors and safety issues of job groupings from the shop floor to the executive suites. We will work with you to establish the levels needed and desired, while recommending appropriate lab and other included services. Appropriate educational offerings shall be offered as indicated.

Laboratory Services - All required laboratory services are provided. The range of services includes drug and alcohol screenings to cardiovascular profiles, and is again based on the needs assessment of your organization. The results of these tests will be confidentially reported only to the appropriate company individuals, and referrals shall be made as indicated. Radiology and audiology services also can be customized to address your needs.

Psychological / Counseling Services - This service is designed for employees who may be experiencing various mental health difficulties, which can include substance abuse problems. This area may also be available to those members of the employee's family who are currently experiencing difficulties in their lives. Within this service, complete psychological evaluations are offered, along with individual, family, and group counseling. Health Index shall refer your employee to the most appropriate Employee Assistance Program when and if the need should arise.

Medical / Legal Consultation - The focus areas for this service are designed to assist employers in the development of legal policies that relate to medical and safety concerns. In addition, Health Index will advise and assist you in evaluating injury claims and past medical histories, current treatments, job duty restrictions, and return-to-work evaluations.

PROVISION OF SERVICE:

Due to the ever-increasing healthcare costs of today, Health Index will develop a customized Program with a fair market value that can easily be adopted into a company's budget. Because Health Index believes in being "customer friendly" and "service oriented," all corporate services Previously identified may be provided on-site at your company's location, or at one of offices.

TIME OF SERVICE:

Health Index will schedule its personnel to be present at your location at a time that is convenient for both the employer and employee. In its provision of services, Health Index will customize a program to meet both the fiscal and time-constraint needs of the company. Health Index is also available for time-sensitive services as determined by the specific needs of your company.

PERSONNEL:

Health Index maintains the services of licensed physicians, registered nurses, medical Technologists, counselors, and social workers. Our corporate team's healthcare administrative experience totals over 70 years of demonstrated strengths in operations, strategic planning, human resources, physician practice, finance, delivery of care, quality assurance, and education.
