Physical examinations also may be scheduled on day or evening shifts; however, prescheduling is required. SINGLE POINT OF CONTACT Health Index, Inc. will provide testing services on a 24-hour basis and will maintain a single point of contact with the employer. We will provide you with appropriate information to ensure quality of services to your organization. SCHEDULING OF SERVICES Health Index, Inc. is a service-oriented corporation. Our services are designed to meet your needs. Scheduling that is suitable to your organization shall be done during normal business hours between your representative and our helpful staff. PHYSICAL EXAMINATIONS Health Index, Inc. provides several classifications of physical examinations, giving your business a menu of choices from which to choose the examinations you require. We will assist you in selecting among options that include:       Basic Screening Physical Examination       CDL Examination       Return-to-Work Examination       Injury / Wellness Follow-Up Examination       Executive Physical       Painter's Physical       Lead Physical Basic Screening Includes: Health History: seizures, tuberculosis, cardiovascular disease, diabetes, ulcers, psychiatric disorders, asthma, kidney disease, permanent physical defect, nervous disorder, orthopedic injuries, hospitalizations, obstetric history, immunizations, allergies, last examination date, family history, chronic illness. Physical Examination: general appearance and development, review of body systems, skin, hair, head, eyes, ears, nose and sinuses, mouth and throat, neck, breast, axilla, respiratory system, peripheral vascular system, gastrointestinal system, urinary system, musculoskeletal system, neurologic system, hematologic system, endocrine system. Functional Assessment: activities of daily living, self-esteem / self-concept, activity / exercise, sleep / rest, nutrition / elimination, interpersonal relationships, coping and stress management, personal habits. LABORATORY SERVICES All required laboratory services are provided, including drug and alcohol screening, on-site with 24-hour coverage. This also includes radiology and audiologist services. Additionally, we will discuss individual company needs with you related to pre-employment and periodic testing, which may also include random drug screening if that is your company's desire. Health Index will further assist you in policy support. FEE FOR SERVICES Health Index, Inc. is reimbursed for services on a case-by-case basis for services rendered. There is no up-front retainer or registration fee charged. ADDITIONAL SERVICES OFFERED Health Index, Inc. is capable of customizing our services to meet your organizational needs. If your business requires a service that has not been discussed, Health Index will review your needs and customize a service to meet them. EDUCATION Health Index, Inc.'s clinical staff maintains certifications as instructors in the areas of CPR, First Aid, and Drug and Alcohol Symptoms Recognition—to only name a few. We can provide additional training to company employees upon request.
